The importance of correct testosterone levels

Many overweight men have low levels of testosterone.
For some, testosterone becomes so low that it can become a health risk. In addition, many men with low testosterone experience a lower quality of life as testosterone affects both mental and physical health.

There are many good reasons to keep track of your testosterone levels, from sexual health to stamina in everyday life, motivation and mental well-being. Thankfully, testosterone levels often rise in connection with weight loss and you will feel that you have your life and drive back.

Testosterone deficiency - hypogonadism, can lead to a host of symptoms such as decreased sex drive, decreased fertility, osteoporosis and decreased muscle mass/strength, to name a few. This is a simple and quick test used to identify testosterone deficiency in men. The test consists of 10 questions relating to symptoms of testosterone deficiency such as decreased libido, fatigue, decreased muscle mass and mood changes. A good start is to complete the 10 yes/no questions in the ADAM test. You will then receive an evaluation. It is unlikely that you have a testosterone deficiency.

Generally, 8-12 in testosterone is considered a gray area. If you have symptoms, you can be treated. Under 8 are almost always treated. However, you start with a weight loss to see if the testosterone rises. After a new test, an evaluation is made by a doctor. We set the right dose in consultation with the patient. Risks of testosterone deficiency and obesity

Men who are overweight or obese are at greater risk of testosterone deficiency for several reasons:

Adipose tissue and testosterone:
Adipose tissue has an enzyme called aromatase, which converts testosterone into oestrogen (the primary female sex hormone). When a man has a lot of adipose tissue, this conversion can lead to reduced levels of free, circulating testosterone.

Insulin resistance:
Men who are overweight or obese often have insulin resistance, which can negatively affect testosterone production.

Inflammation:
Overweight and obesity can also lead to chronic low-grade inflammation, which in turn can affect the function of Leydig cells in the testicles, which are responsible for testosterone production.

What testosterone deficiency means for obesity

Decreased testosterone levels in obese men can worsen the symptoms of obesity. Testosterone deficiency can lead to reduced muscle mass, increased fat accumulation and reduced metabolism. These changes can in turn worsen obesity, creating a vicious circle.

Which medications do you use for weight loss?

We usually use the very latest type of medication, known as GLP-1 analogues, but the doctor always makes an individual assessment to determine which treatment is best suited for each individual.

Do I have to take medication for my obesity for the rest of my life?

Obesity is a chronic disease. Unfortunately, when you remove the drug, you are often back where you were before you started and the body starts striving for its highest weight again. It is not dangerous to stop taking the medication and then start again if you notice that you begin to gain weight. With lifestyle changes such as an increase in exercise and a different diet some may achieve an energy balance after reaching a healthy weight, and may therefore not need to go back on the medication.

Who can receive medication for overweight/obesity?

Many different types of anti-obesity medicines have been tested in clinical trials. These medications are recommended for use in cases of obesity (BMI ≥ 30 kg/m2) or overweight (BMI ≥ 27 kg/m2) with complications or risk factors present.

These risk factors may include high blood pressure, elevated blood lipids, elevated blood sugar, atherosclerosis, or sleep apnea syndrome.

Medications can also be used to maintain weight loss, for example, in individuals who were previously severely overweight and have successfully lost weight through methods like dieting or weight loss surgery.

What side effects may I experience?

Because it is the body's own hormone that is used, which has only been modified to slow down its breakdown, the side effects are few and mild. In the larger studies that have been conducted, the side effects are relatively similar between  test groups taking placebo and test groups taking medicine. Side effects are most common at the beginning of treatment, which is why the initial dose is low and increased slowly.

You may experience nausea, gas, and diarrhoea, mild gastritis, or constipation, but with the support of a dietitian, most symptoms can usually be avoided.
